A CRITICAL ANALYSIS OF THE ROLE OF FASTING IN CLINICAL ONCOLOGY AND AGEING: DISPELLING A MYTH

Caloric restriction and fasting are the only non-pharmacological interventions extending lifespan and reducing cancer risk in mice, with evidence suggesting fasting may improve cancer therapy efficacy. Are we convinced this is reality, or just hype driven by business interests?

The impact of dietary restriction—including caloric restriction, intermittent fasting, and fasting-mimicking diets—on the health of cancer patients, as well as on the enhancement of well-being and the extension of maximum lifespan in animal models, has recently attracted significant attention. Despite the uncertainty surrounding the underlying mechanisms and causal relationships between these dietary interventions and their observed benefits, the topic has been widely discussed within both the scientific community and social media platforms. This widespread discourse raises concerns regarding the accuracy of data interpretation and the potential for substantial misrepresentations. A recent study demonstrated that glucagon agonists can extend the lifespan of aged mice; however, this effect appears unlikely to be directly related to fasting periods, as the animals consumed reduced amounts of food throughout the day without undergoing fasting intervals exceeding 12 to 16 hours. Accordingly, this review aims to offer an alternative, more reasoned interpretation of the existing preclinical and clinical literature on fasting, providing a balanced perspective to anchor the discussion in empirical evidence.
