Open Access Research and Humanitarian Health: Supporting Research Without Borders

Médecins Sans Frontières (MSF), also known as Doctors Without Borders, is a humanitarian organisation that has been providing medical care in more than 75 countries. Alongside it’s frontline humanitarian work, MSF plays an important role in advancing research and discussion in global health.
Every year, MSF Scientifics Days bring together those interested in global and humanitarian health. MSF describes them as “conferences without borders” creating a space for healthcare professionals, researchers, humanitarian workers and policy makers to gather and have meaningful discussions about the work of MSF.

At this year’s conference, some of us at BMC in London had the opportunity to attend sessions and engage with a wide range of global health discussions, highlighting topics such as malnutrition and trauma care in Gaza, malaria vaccines, novel snakebite antivenoms in Ethiopia, maternal healthcare in Pakistan and Sudan, and new approaches for HIV care.
Beyond the scientific and clinical presentations, MSF hosted a panel discussion on medical ethics and how MSF’s research is used. Although a variety of topics were discussed, a common theme emerged throughout: the importance of open access research in supporting frontline healthcare workers responding to crises around the world.
The Importance of Open Access in Humanitarian Health
Open Access is a publishing model which makes research accessible to all, with no subscription fees and no institutional barriers, meaning articles are freely available to anyone anywhere, not just scientists with a Journal subscription. Open Access research allows for collaboration and dissemination of medical data and information, all of which is vital in the context of humanitarian crises, epidemics, natural disasters, and healthcare in conflict settings.
MSF not only advocates for open access publishing, but it also champions open access research; all studies conducted by MFS Operational centres, are openly accessible on their Research Management and Impact Tool (ReMIT). “ReMIT is open access to improve transparency and accountability of MSF research”. Additionally, MSF has their own digital library called MSF Science Portal, where thousands of articles and conference materials relating to MSF’s work are all openly accessible online.
Removing Barriers in Open Access Publishing
Open Access publishing is typically supported through an Article Processing Charge (APC), which helps to cover the costs associated with publishing the article, including production, integrity and long-term accessibility. While the open access model increases accessibility for readers, it can present a financial barrier for some authors, particularly in low- and middle-income countries – and several publishing companies provide assistance to help overcome these barriers.
As the first commercial open access publisher, founded in 2000, BMC recognises the importance of supporting equitable participation in publishing. BMC offers waivers and discounts for eligible authors and has established open access agreements with institutions around the world to support broader participation in research publishing.

Through sharing Behind the Paper blogs and keeping up with News and Opinion content, the Research Communities not only provides researchers with the opportunity to connect with others in their field but also engage with wider conversations and perspectives.

Launched in 2013, BugBitten is a blog for the parasitology, vector biology and global health community, providing a key space for researchers to share personal stories, discuss emerging topics and highlight developments across infectious disease research. Racism and Health
The ongoing advancement of our understanding of how racism impacts health - both directly and structurally - is crucial for addressing the deep-rooted health inequities that persist globally. Structural racism shapes health outcomes among racialized populations, contributing to inequities in healthcare access and quality, and impacting social, political, or environmental determinants of health. Existing research also connects direct and visible manifestations of racism, such as major and everyday discrimination, to negative health outcomes. At the same time, racism does not operate in isolation. A growing body of research shows how it intersects with other systems of oppression—such as classism and patriarchy—and with broader social determinants of health, producing compounded and context-specific inequities. An intersectional lens is therefore essential to understand how overlapping structures of power and exclusion shape health and healthcare experiences. These insights are essential in conducting rigorous and thorough health research, guiding anti-racist health policy, and ultimately, ensuring equitable health for all.
Looking forward, research holds the potential to develop new methodologies and indices and unveil complexities in how racism manifests within healthcare systems and societal structures. By deepening our collective understanding, we can develop more effective interventions that target the root causes of health inequities.

Climate and Environmental Change as Drivers of Infectious Disease: A Planetary Health Perspective
Human health is increasingly shaped by the interconnected pressures of climate change, environmental degradation, and socio‑ecological disruption. These planetary‑scale changes influence not only the epidemiology of infectious diseases, but also the public health systems, social structures, and community capacities that determine exposure, vulnerability, and resilience. This Collection adopts a planetary health perspective to explore how climate variability, ecosystem disruption, and human–environment interactions affect infectious disease risks and the public health responses needed to address them.
We welcome research that examines how shifts in temperature, precipitation, humidity, extreme weather events, land‑use change, biodiversity loss, and urbanization influence pathogen ecology, transmission pathways, and population vulnerability. Beyond epidemiological patterns, the Collection seeks contributions that address preparedness, surveillance, health system resilience, risk communication, governance, and cross‑sectoral interventions aimed at mitigating and adapting to climate‑related infectious disease threats.
Studies leveraging epidemiological datasets, climate and environmental records, spatial mapping, and predictive modeling are encouraged, as are qualitative, policy‑focused, and implementation‑oriented works that illuminate barriers and opportunities within public health systems. Particular attention is given to inequities in exposure and resilience, the needs of high‑risk populations, and the role of community‑level and institutional adaptation strategies.
By integrating epidemiology, environmental science, and public health practice, this Collection aims to advance understanding of climate–disease interactions, strengthen early warning and response systems, and inform adaptive strategies that protect and promote planetary health in a rapidly changing world. Topics of interest include but are not limited to:

- Impacts of extreme weather events and environmental disruption on disease risk
- Public health preparedness, surveillance, and early warning systems

Open access is not merely a publishing model it is a recognition that knowledge is a human right, not a commodity. In my recent health research, I apply the rule (B+F=Nf):
(B) is biological truth: the body, the disease, the blood. This is what MSF treats daily in the field.
(F) is internal witnessing: the patient witnessing their pain, being recognized as a human, not just a clinical case.
(Nf) is the narrative trace: the published research, the told story, the changed policy.
When MSF opens its research to all, using ReMIT and the Science Portal, it does not merely share information. It grants witnessing (F) to patients and communities, transforming their suffering into a trace (Nf) that changes policies.
This is truly research without borders: not only without geographic borders, but without borders between the body (B), witnessing (F), and the trace (Nf).
