Lumbar epidural haematoma after acupuncture: a case report and review of the literature

Case Summary

An 80-year-old Chinese man developed progressive right-sided lumbosciatalgia and foot drop 10 days after undergoing three sessions of electroacupuncture and an L5–S1 paravertebral nerve block for chronic low back pain. Magnetic resonance imaging demonstrated a large organized L5–S1 spinal epidural hematoma causing severe compression of the thecal sac and S1 nerve root. The patient underwent urgent decompressive laminectomy, evacuation of the hematoma, and cement-augmented pedicle screw fixation, resulting in complete neurological recovery at 3 months. This case highlights the rare but potentially devastating risk of spinal epidural hematoma following acupuncture and emphasizes that new or progressive neurological symptoms after needling should prompt immediate MRI evaluation and timely surgical intervention when significant neural compression is present.
